Long-term conditions such as diabetes, high blood pressure and high cholesterol often have few symptoms – which is exactly why regular follow-up matters.

A personal care plan

Your general practitioner will work with you to create an individual plan: the right medicines, sensible targets, lifestyle advice and a schedule for check-ups and tests.

Make the most of each visit

  • Bring a list of all medicines and supplements you take
  • Note any new symptoms and when they started
  • Ask about your test results and what they mean

When to get help sooner

Contact your doctor if your readings change suddenly or you feel unwell. For chest pain, difficulty breathing or other emergency symptoms, call 998 immediately.

Our doctors at Bright Care Clinic provide chronic disease follow-up every day from 8 AM to 11 PM.
